Edward Z. Baker

LAKE BUTLER - Edward Z. Baker of Lake Butler died at his home Friday. He was 58.

Mr. Baker was born in Arden, N.C., and lived in Alachua and Union counties for the past 20 years. He was a truck driver for Tarmac America Inc. for the last several years and was a member of Blackwelder Memorial Freewill Baptist Church in Worthington Springs.

Survivors include his wife, Jackie M.C. Baker of Lake Butler; one daughter, Julia Cannon of Hartsville, Tenn.; and two sons, Barry Baker of Atlanta. and Joey Baker of Lake Butler; and two grandchildren.

Expressions of sympathy may be made as donations to Hospice of North Central Florida.

Thelma Adele Campbell

Thelma Adele Campbell of Newberry died Sunday at Ayers Health and Rehabilitation Center in Trenton. She was 79.

Mrs. Campbell was born in Baltimore and moved to Newberry from Boring, Md., in 1992. She was a retired licensed practical nurse and was a church organist. She was a senior citizen hostess at the McDonald's restaurants on Newberry and Archer roads.

She is preceded in death by her husband, Christopher Columbus Campbell; one daughter, Joan Annette Evans; and one grandson, Brian M. Evans.

Survivors include two daughters, Nancy Estella Grogan of Newberry and Jacqueline S. Campbell of South Hampton, N.J.; seven grandchildren; and one great-grandchild.

Expressions of sympathy may be made as donations to Hospice of North Central Florida.

Andrew Jackson Eaves III

STARKE - Andrew Jackson "Jack" Eaves III of Starke died Saturday at Shands at the University of Florida. He was 67.

Mr. Eaves was born in San Antonio, Texas, and was a longtime resident of Starke. He was a retired automotive foreman for the Florida National Guard, a member of First United Methodist Church, and an avid fisherman and golfer.

Survivors include his wife, Carolyn F. Early Eaves of Starke; two daughters, Andrea Eaves Waterhouse of Starke and Carmen Eaves Forsyth of Starke; two sons, Andrew Jay Eaves of Lawtey and Len Eugene Eaves of Starke; two sisters, Betty Wroten of Groveland and Patsy Wilkison of New Braunfels, Texas; two brothers, Robert Eaves of Loxahatchee and Dr. Ronald Eaves of Opelika, Ala.; nine grandchildren; and three step-grandchildren.

Expressions of sympathy may be made as donations to First United Methodist Church of Starke, Box 157, Starke, 32091, or to a charity of the donor's choice.

Michael Phillip Insalaco

Michael Phillip Insalaco of Carrollton, Texas, died Aug. 26. He was 43.

Mr. Insalaco was born in the Bronx, N.Y., and lived in Gainesville before eventually moving to Carrollton, where he was an electrical engineer with Dallas Semiconductor Inc. He was a Catholic.

Survivors include his parents, Horace J. and Angelina "Mickie" Insalaco of Gainesville; one sister, Maryann Insalaco of Jacksonville Beach; and three brothers, James Insalaco of Winter Park, Vincent Insalaco of Little Rock, Ark., and Steven Insalaco of Palm Harbor.

Expressions of sympathy may be made as donations to the St. Patrick's Catholic Church School Building Fund, 500 NE 16th Ave., Gainesville, 32601.

Mildred Odom Smith

Mildred Odom Smith of Lake City died Friday in Maryville, Tenn., following a brief illness. She was 71.

Mrs. Smith had lived in Lake City for 40 years. She was a homemaker and a member of Mount Carmel Baptist Church.

Survivors include three sons, Leslie Odom of Alachua, Richard Odom and Rodney Odom, both of Lake City; two daughters, Margaret Bagley of Branford and Sharon Suggs of Gainesville; fourteen grandchildren; and 12 great-grandchildren.

Doris Jean Burford Wagner

Doris Jean Burford Wagner of Gainesville died Saturday at Hospice House following an extended illness. She was 52.

Mrs. Wagner was born in Charleston, W.V., and moved tohas lived in Gainesville in 1963. She was an engineering assistant at BellSouth and a member of the Telephone Pioneers of America. She was a Protestant.

Survivors include her mother, Dincy B. Burford of Gainesville; two daughters, Jeanne Wagner and Kristi Wagner, both of Gainesville; and two brothers, Ed Burford and Randy Burford, both of Gainesville.

Expressions of sympathy may be made as donations to Hospice House or the American Cancer Society.

Thomas M. Bailey

TRENTON - Thomas M. Bailey of Trenton died Thursday at home. He was 64.

Born in Salem, Va., Mr. Bailey moved to Trenton from Largo 11 years ago. He was an electrician.

Mr. Bailey was Protestant.

Survivors include his wife, Suzanne Bailey; four sons, John Bailey, Thomas M. Bailey Jr., William Mosher and David Mosher; two daughters, Debra Bailey and Suzanne Harmeier; sisters, Mary B. Bailey and Susan Bailey; and three grandchildren.

Bonnie Lou Ballew

O'BRIEN - Bonnie Lou Ballew died Saturday at her residence in O'Brien. She was 55.

She was born in Eaton Rapids, Mich., lived in Bell for 12 years and Beachville for eight years.

She was a homemaker.

She was a member of Pleasant Hill Baptist Church.

Survivors include her husband, David Ballew of Beachville; a son, Ronald L. Richardson of Michigan; two daughters, Dena M. Thomas of Illinois and Tracy Brown of Michigan; two sisters, Lois Larkin of Michigan and Marilyn Napier of Branford; and a brother Kennith C. Ward Jr. of Beachville.

Mary Olive Bell

CROSS CITY - Mary Olive Bell of Cross City died Sunday at the Tri-County Nursing Home at Fanning Springs. She was 91.

She was born in Lee and lived in Dixie County for most of her life.

She was a homemaker.

She was a member of McCalls Chapel of Branford.

Survivors include a daughter, Faye Cannon of Cross City; two sisters, Yoland Buckhalter and Jean Ergle, both of Gainesville; a grandchild; four great-grandchildren; and seven great-great-grandchildren.

Floyd Ferrell Capo

CROSS CITY - Floyd "Bubba" Ferrell Capo of Cross City died Saturday at Shands at the University of Florida. He was 66.

He was born in Cortez and moved to Cross City 35 years ago. He was a fisherman.

He loved to mend his nets and hunt in his spare time.

He attended the Church of God.

Survivors include his companion, Lousie Lowe of Cross City; three daughters, Debra Gruelle of Pine Island, Amy Potter of Cross City and Lucille Miller of Long Boat Key; two sons, Floyd Ferrell Capo Jr. of Chiefland and David Capo of Cross City; two sisters, Jewelle Alkine and Sandra Clarkson, both of Crystal River; and 10 grand-children.

Raymond Francis Gillette

Raymond Francis Gillette of Gainesville died Sunday at Shands at AGH. He was 54.

Mr. Gillette was a retired UPS manager and a Vietnam War veteran, serving in the U.S. Navy.

He was born in Perth, Australia, and moved 30 years ago from Jacksonville to Gainesville.

An active fisherman and hunter, Mr. Gillette was a member of the National Rifle Association and the Skeet Club of Gainesville.

He also belonged to the Church of England.

Survivors include his wife, Martha Arroyo Gillette of Gainesville; two daughters, Larae MacKinnon of Gainesville and Patti Crews of Alachua; two sons, Raymond Gillette and Michael Gillette, both of Gainesville; his mother, Ruth Gillette of Jacksonville; his brother, John Gillette of Tampa; his sister, Leah Mershon of Jacksonville; and one grandson.

Expressions of sympathy can be in the form of contributions to Hospice of North Central Florida or to Ducks Unlimited, 1 Water Fowl Way, Memphis, Tenn. 38120.

Cassidy Ann Lingerfelt

Cassidy Ann Lingerfelt of Gainesville died Monday after a short illness. She was 18 months old.

She was born in Gainesville.

Survivors include her mother, Sarah Joyce L. Wilkinson of Gainesville; her maternal grandparents, Ann and Buddy Hallowell of Keystone Heights and Boyd and Jean Lingerfelt of Palatka; her brother, Kent Wilkinson of Gainesville; and her sister, Sarah Wilkinson of Gainesville.
